It's Time To Register your child for
Pre-Kindergarten AND Kindergarten for the 12 - 13 school year!
PRE-KINDERGARTEN INFORMATION
Please stop by the Main Office to register. Screening will be held on Friday, May 25th. Please see the schedule below:
Last name ending in A - M, please be at South at 9:00am.
Last name ending in N - S, please be at South at 10:00am.
Last name ending in T - Z, please be at South at 11:00am.
Only children who will turn 4 years old on or before August 31, 2012 will be eligible to register.
*Student Data Sheet (can be obtained in Main Office at South)
*certified copy of birth certificate (can not be mother's copy, hospital copy, or any copy other than the one that has been certified with the raised seal)
*copy of social security card
*Pre-Kindergarten Health Assessment (can be obtained in Main Office at South)
*updated immunization record (must be certified immunization record with NC state seal or if not in NC, a copy from your doctor's office with doctor's information)
*two proof of residences
*drivers license
*Medicaid card (if applicable)
*three consecutive pay stubs
*IEP (if applicable)
*Pre K Application (can be obtained in Main Office at South)
KINDERGARTEN REGISTRATION INFORMATION
Please stop by the Main Office to register. It is very important that you register your child, even if you don't have all the necessary documentation. The number of classrooms we have for next year is determined by the number of registrations we have.
Only children who will turn 5 years old on or before August 31, 2012 will be eligible to register.

You must be in South's attendance zone to register. If you are unsure, please contact the school at 694.1212 or Central Office at 694.4116.

Your child does not need to be present to enroll.

The following documents will be necessary to enroll your child.
*certified copy of birth certificate (can not be mother's copy, hospital copy, or any copy other than the one that has been certified with the raised seal)
*parent/legal guardian photo id (driver's license (along with court custody papers, if applicable)
*proof of residence (current utility bill, property deed, tax bill, or rental lease; driver's licenses will not be accepted)
*updated immunization record (must be certified immunization record with NC state seal or if not in NC, a copy from your doctor's office with doctor's information)
*Kindergarten Health Assessment (must be on the blue NC Kindergarten Health Assessment form; if you have a VA form, the office that did the assessment will need to put on NC form;
*copy of social security card
**If copy of certified immunization record and Kindergarten Health Assessment forms are not received with first 30 days of school, your child will be suspended from school on the 30th day.

Please remember to check to see if your child owes money for before and after school, lunch account, library books, etc. If money is owed, children will not be allowed to participate in extra curricular activities at the end of year.
